Background. Atypical fibroxanthoma (AFX) is a fibrohistiocytic tumor with relatively high local recurrence rates but low metastatic potential. Wide local excision (WLE) and Mohs micrographic surgery (MMS) are common treatments, although no consensus exists regarding optimal therapy. Atypical fibroxanthoma (AFX) AFX is best treated by complete surgical excision, with Mohs micrographic surgery considered the treatment of choice. Metastasis rarely occurs, but there is a high rate of local recurrence, especially in patients who are immunosuppressed. Treatment of atypical fibroxanthoma of the skin (AFX) includes curettage and dessication (C&D), wide local excision (WLE), and Mohs micrographic surgery (MMS).
